NC Childcare Licensing Checklist
To ensure full compliance with NC state childcare licensing laws, please prepare the following items:
- Child’s original birth certificate
- Up-to-date N.C. immunization record
- N.C. health assessment form (signed by physician)
- Completed emergency contact & medical form
- Custody or guardianship paperwork (if applicable)
- Allergy or medical action plans (prescribed by doctor)
- Center-specific enrollment acknowledgments
